At Green Iowa Medical Cannabis Clinic, we strive to make getting an evaluation for the Iowa Medical Cannabidiol Program as easy as possible.

First, sign up for and schedule an appointment at the button below and pay the evaluation fee.  You will receive an email and text confirmation of your appointment.  You should also receive a reminder the day before your appointment.

W do online telemedicine evaluations, but we are excited to begin doing in-person evaluations for certification at our office in Apex Health at 2925 Ingersoll Ave, suite 102, in Des Moines!

On the day of your appointment, you will receive an email and/or text notifying you of your evaluation locations.  

For telemedicine evaluations, you will click the link to join the provider in a video conference.  You will have your evaluation at that time.  For in-person evaluations, you will go to our office at 2025 Ingersoll Ave, suite 102 in Des Moines.  We recommend showing up 5 minutes before the start of your appointment.

After the evaluation, if you qualify for the Iowa Cannabidiol Program, the provider will email you the completed form and instructions on how to register for the program with the state of Iowa.
