WHAT IS THE COST OF AN EVALUATION?
Currently, evaluations cost $100 for a 15 minute Telehealth or telephone meeting.
AFTER AN EVALUATION, HOW DOES A PATIENT GET THEIR CANNABIS CARD?
After getting an evaluation and receiving a certification form from the provider, the patient will have to apply to the Iowa Department of Public Health either online or by mail.
Required forms and more information can be found at:
AFTER MY EVALUATION, HOW WILL I RECEIVE MY CERTIFICATION PAPERWORK?
After having your evaluation with Green Iowa and being found to have a qualifying condition, Green Iowa will email you a PDF of your paperwork for the application along with a form outlining instructions on submitting your application to the Iowa Department of Public Health.
If you would prefer or if it is more convenient, your form may be mailed via USPS.
CAN I GET A MEDICINAL CANNABIS CARD IF I HAVE A MEDICAL DISORDER FOR WHICH CANNABIS MAY BE AN EFFECTIVE TREATMENT, BUT IS NOT ON THE LIST OF QUALIFYING CONDITIONS?
Unfortunately, no. There is no allowance for provider discretion in cannabis treatment. Providers are only allowed to suggest medicinal cannabis for patients who have a qualifying condition according to Iowa law.
HOW DO I RENEW MY CERTIFICATION?
By Iowa law, a patient certified by their provider for the Iowa cannabis program must have their certification renewed annually. This means that cannabis patients must be reevaluated annually.
This evaluation is essentially the same as the initial evaluation and all required payments for application to the Iowa Department of Public Health are the same.
